Job Description

GCBHS has immediate openings for Case Managers/ Youth Transition Facilitators to provides Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill youth and young adults. CPST services are designed to provide specific, measurable, and individualized services to each person served. We are looking for BSW or Psychology or Sociology or Criminal Justice or related Human Services degree graduates. Responsibilities for Case Manager/TF:

  • Provides case management - Community Psychiatric Support Treatment (CPST) services and Therapeutic Behavioral Services (TBS) to youths and young adults with severe mental illness in an outpatient facility and in the community
  • Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include: living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, social supports, leisure and recreation
  • By assertive outreach, attempts to engage the client in identifying and expressing his/her own needs, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis
  • Gains knowledge of the following: mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client's use of services, and applicable costs of use, and the opportunities for alternative resources.
  • Strives to understand the characteristics of the client population and practice setting in which this population is served; the socioeconomic, cultural, gender, racial, and sexual orientation issues that may affect the differential use of services; the fiscal impact and consequences of using particular services; and the system of agency-community resources and the interrelationships among system components

Requirements for Case Manager/TF:

  • Masters degree in mental health, vocational training, education, social work, counseling
  • OR BSW (Bachelor of Social Work), Bachelor in psychology, criminal justice, or a related human services field plus two years of specialized education, training, or experience in one of the specified fields of study
  • Experience in social services and behavioral health preferred but not required
  • Position requires frequent driving of own personal vehicle. A valid driver's license, vehicle, and insurable driving record are required
